Submit your Claims

Once everything has been put together - and all your claims forms have their fields filled - you will need to review them.
The process is to make sure that you have everything required with correct information. Mistakes or missing
documentation can cause lengthy delays in any of your claims and create complications.

You can submit your claim via mail or fax once complete to the addresses provided. We strongly recommend making
copies of all the paperwork and documents in your submission in case something goes missing. Our advice also includes
any mailing receipts for your convenience. We take this opportunity to reiterate - your filed claims must arrive within 90
days from the service date.

    The claims process can take between 15-20 days, noting it could be longer in certain circumstances. Once your claim is processed, you will receive an Explanation of Benefits (EOB).
